Use this form only if you are unable to clear your cubby and/or return your access fob before your membership ends.

You may still complete your membership cancellation through this alternative process by the same cancellation deadline. The following fees will be deducted from your security deposit:

• $20 cubby cleaning fee

• $20 fob replacement fee

• Total: $40

Since membership fees are billed on the first day of each month, this means the cancellation process must be completed by the last calendar day of the month to avoid the following month’s charge.
For example, to avoid being charged on July 1, all three steps must be completed by June 30.

Membership fees are not prorated or partially refunded.
